First Visit

Prior to your first visit, we ask that you complete the Patient Information and Health Forms which will be mailed or can be downloaded from our website (see Patient Forms below). Also please bring any insurance information that may be necessary and any dental x-rays taken within the last six months.

Our initial examinations are a courtesy to our new patients. This allows all individuals to become educated as to their orthodontic situation and to feel comfortable with their decision for care. We also feel comfortable in that we have provided you with the knowledge to make a good decision.

We will address the following questions that cover the basics of orthodontic treatment. We encourage you to ask questions as well.

  • Is there a condition that orthodontics can address?
  • Is treatment needed now or should treatment be delayed until appropriate growth, tooth eruption or other factors have occurred?
  • What treatment procedures will be used to correct the problem?
  • Do any permanent teeth need to be removed?
  • How long will treatment take?
  • How much will it cost?
  • What are my payment options?

Typically your first visit to our office will require one hour of your time. At the initial visit, our Treatment Coordinator will meet with you and address your concerns and describe our treatment protocol. Dr. Crane or Dr. Seager will then conduct a complete examination and diagnosis, and provide an explanation of our findings. We will require clinical records to thoroughly evaluate your needs. At this time, we will also make a recommendation for treatment and we will discuss any concerns you may have.
